1. High School Report Card (Form - 138)
2. Photocopy of NSO Authenticated Birth Certificate
3. Certificate of Good Moral Character signed by the Principal/Adviser/Guidance Counselor
4. Examination Result (Guidance Counselor's Office)
5. 3 copies of latest (2 X 2) pictures
6. 2 copies of (1 x 1) ID picture

B. Transferees

1. Transfer Credential/Honorable Dismissal
2. Transcript of Record for Evaluation purpose
3. Certificate of Good Moral Character signed by the Dean/Guidance Counselor/Barangay Captain
4. Photocopy of NSO Authenticated Birth Certificate
5. 3 copies of latest (2 x 2) pictures
6. 2 copies (1 x 1) ID picture

1. Proceed to the Guidance Office to get information about the course you wish to enroll.
2. Pay the testing fee at the accounting office.
3. Take the Entrance Examination.
4. Secure the result of entrance examination at the Guidance Office and submit the requirements then proceed to the respective head for the final interview and schedule of classes.
5. Present the pre-registration form signed by the department head at the registrar's office for encoding.
6. Pay at the accounting office

1. Proceed to the Guidance Office to get information about the course you wish to enroll.
2. Pay the testing fee at the Accounting Office and present the receipt at the Guidance Office for the exam schedule.
3. Take the Entrance Examination.
4. Secure the result of entrance examination from the Guidance Office and submit the requirements, proceed to the respective department head for the final interview and schedule of classes.
5. Present the pre-registration form signed by the department head at the registrar's office for encoding.
6. Pay at the accounting office.
